Like we really needed it, we see again that Carl (Chandler Riggs) was indeed bitten by a zombie in the midseason finale, while rescuing newcomer Siddiq (Avi Nash) in the forest. Showrunner Scott M. Gimple in particular has been the subject of a tremendous amount of blow-back when it comes to the decision to kill off Carl, a character who does not die at this point in the comics.
The producers don't explain it, and they've left us to wonder.

We were seeing this lovely dream sequence - Negan was being nice to Judith and then they cut to Rick underneath this tree and his hand is all bloody.
The episode strengthened those sequences by juxtaposing them with the death of one of Negan's top henchman, in another test for the character of Morgan (Lennie James), who has gone from resisting violence to ruthlessly if grudgingly embracing it. That's probably not the case though. Outside of Rick and Michonne, no one else even goes near Carl. Who could forget when Andrew J. West was signed as a series regular in season five after making his first appearance in the season four finale - and his cannibal king Gareth lasted a grand total of three episodes before Rick embedded a knife into his head. He must grapple with the loss of his own son, the guilt that no doubt already plagues him and that, in order to grant his child's dying wish, he has to find a way to make peace with Negan and end the war. That's the current main theory that fans are circulating, but we'll add more theories to this story as they are developed.
